1. Download form from the website

For research on rodents, download

For research on both living and non-living aquatic species, download

For research on wild animal or research that does not require animal husbandry, download

For research conducted in other institute, download

2. Forms that are received by the 5th of every month will be reviewed for approved by IACUC in the same month.

3. To submit a protocol, send PDF file to email (musc.iacuc1@gmail.com).
